Business Enviromental Survey
EPA and DEQ regulations require us to regulate certain businesses that discharge
substances into the sewer system that could damage the treatment system or pass through the system and into the Willamette River. This survey is part of our effort to identify those businesses.

No, we’re surveying businesses, not locations. Please fill out the survey form to the best of your ability and return it to us.
Yes. We need a signed form for each business for our records.
We may not have received the first one, or your survey and the second notice letter may have crossed in the mail. Email or phone us to confirm that we received your survey. (Our contact information is on the survey form).
Yes. Either is acceptable as long as the survey form is completed and signed.
It may be that a previous occupant of your home ran a business out of their residence. Or we may have made a mistake in addressing the survey. Contact us by phone or email and we will take care of this.
If you received only a survey addressed to the other business, fill out the survey with your business information and return it to us, even though it wasn’t addressed to you. Note on the form that the business the survey was addressed to is no longer at that address.
If you received one survey addressed to your business and second one for the other business, fill out the first and write “closed” or “moved” on the second, and send both to us.
